Default Is This the Right Form to Register a Trade Name (DBA)?

Is the form on the link i posted the right form for DBA? If i have no employees and i have insurance and am bonded do i still need to register as LLC? i would rather not at this time so will the form in the link be ok as a substitute for both?

That looks as though it would be the form to use for registering a DBA ("trade name") in the state of Ohio.

I don't believe you're required to register as an LLC. According to the IRS, if you operate your business yourself (i.e. not a partnership) and you have no employees, you're considered a sole proprietor and you can simply file your business income taxes using Schedule C as part of your personal tax return. (State sales taxes, if applicable, would be another matter, filed through the state.)

There are good reasons, both tax and business, for organizing as an LLC (or partnership, or C-corp, or standard corporation, or LLP, or whatever). There are also good reasons for NOT organizing as any of these things and simply operating as a sole proprietor (if that's applicable to your business to start with).

As the state/local tax, registration and business licensing regulations differ from one place to another, there's no way anybody here can tell you for sure what might or might not be required in your jurisdiction. As we don't know the details of your personal tax and business situation, we can't intelligently advise you as to what sort of business organization would be the most beneficial to you.

I strongly advise you to consult with a qualified tax professional and/or a legal advisor to determine what licensing and other regulations might apply to your particular kind of business and what kinds of tax implications there might be for your specific situation. Then you can make an informed decision as to what's the best way to proceed.
